Description
Oakwood Village has an exciting opportunity for qualified candidates to join our Prairie Ridge (East Side of Madison) senior living community facility team as aLPN Shift Coordinator. Our LPN Shift Coordinator is responsible for providing service to the residents in compliance with appropriate codes, including emergency medical needs with emphasis on promoting wellness and independence. This position also provides overall supervision of Resident Assistants in assisted living.

The LPN Shift Coordinator position has one shift available:
- Full Time, 80 hours every two weeks on the PM shit from 2:45pm-11:15pm with every other weekend off!

Job Responsibilities of a LPN Shift Coordinator (Not intended to be an all-inclusive list)
1.Responds to symptoms of change in mental or physical conditions of residents, provides services to residents that meet their physical, emotional, intellectual, social and spiritual needs, and reports these changes to the Administrator/Nurse Supervisor/Primary Care Provider/Health Care Power of Attorney (HCPOA) as needed.

2.Overall supervision and leadership of Resident Assistants and other staff when necessary, promoting teamwork and the policies of Oakwood. Follows Oakwood mission.

3.Obtains and transcribes physicians orders as needed. Communicates orders to Pharmacy. Reports any need for follow-up to oncoming staff. Communicates changes to resident and HCPOA as needed.

4.Supervises Resident Assistants in passing and recording the administration of scheduled and PRN medications and treatments as needed. Reports medication errors to the Administrator/Nurse Supervisor and fills out appropriate forms.

5.Maintains and monitors direct supervision of all Resident Assistant Staff in Assisted Living.

6.Maintains records and charts reflecting resident's condition. Oversees charting responsibilities to Resident Assistants as needed.

7.Provides assistance to residents with Activity of Daily Living (ADLs) to promote independence on a daily basis as part of the Oakwood team.

8.Reports significant resident incidents or injuries to physician and responsible person as they occur. Then sends follow-up reports to the Administrator/Nurse Supervisor.

9.Attends appropriate staff development sessions for development and growth. Attends staff meetings and other meetings as assigned.

10. Participates in discussion and development of resident's individual care plan/ISP; assures that Resident Assistants are familiar with and providing services/support as indicated on each resident's care plan/ISP. Updates (Individualized Service Plan) ISP on an ongoing basis.

COMPETENCIES

1. Maintains communication with Administrator/Nurse Supervisor and Program Director as well as families/responsible persons.

2. Communicates to Administrator/RN Manager when residents need to be transferred to a more appropriate level of care or setting.

1. Possess a genuine caring and ability to relate well to the residents.
2. Knowledge and skill in current RAI/II practices, especially relating to geriatric population.
3. Ability to plan, organize and prioritize workload.
4. Ability to build and maintain teamwork relationships with other staff.
5. Ability to effectively lead and supervise the Resident Assistants.
6. Ability to perform job responsibilities in a professional, accurate, and conscientious manner.
7. Ability to maintain and protect the confidentiality of information.
8. Ability to exercise independent judgment and make sound decisions.
9. Ability to provide leadership that fosters effective teamwork in meeting the mission of Oakwood
10. Willingness to work every other weekend, holiday and on-call schedules
11. Knowledge of all appropriate codes.

EXPERIENCE, EDUCATION, AND/OR TRAINING
1. Licensed as an LPN in the State of Wisconsin.

2. 2 years of prior experience of LPN and Supervisory preferred.

3. CPR certified and CBRF certified preferred.
